The process begins with a conversation. We take time to understand the patient’s condition, recent hospital stay, current challenges, and recovery goals. We also explain how home health works, answer questions, and help patients and families understand what services may be available.

A licensed clinician completes a comprehensive in-home assessment to review the patient’s medical condition, medications, mobility, safety concerns, and daily care needs. Our team also coordinates with the patient’s physician to confirm eligibility and establish the appropriate level of care.

Every patient receives an individualized care plan based on their diagnosis, recovery goals, and physician’s orders. The plan may include skilled nursing, physical therapy, occupational therapy, speech therapy, medication management, wound care, and patient or caregiver education.

Care continues through scheduled in-home visits and regular communication between nurses, therapists, physicians, patients, and family members. Progress is monitored closely, and the care plan is adjusted as the patient’s condition, abilities, and recovery needs change.

As the patient improves, our team helps prepare them and their caregivers to manage safely and confidently without regular home health visits. Before services end, we provide education, review ongoing care needs, and coordinate any additional support needed for the next stage of recovery.

The right support at home can make recovery safer and less overwhelming.

Home health helps patients continue healing after illness, surgery, or hospitalization while receiving skilled care in the place where they feel most comfortable.

HOME HEALTH QUESTIONS

Frequently Asked Questions About Home Health Care

Honest answers for patients, families, and caregivers about home health services, eligibility, physician referrals, insurance coverage, and what to expect throughout your recovery at home.

What is home health care?
Home health care provides skilled medical services in the comfort of your home. Depending on your needs, care may include skilled nursing, physical therapy, occupational therapy, speech therapy, wound care, medication management, and patient education under a physician's direction.
Who can benefit from home health care?
Home health is designed for people recovering from surgery, illness, injury, or hospitalization, as well as those managing chronic health conditions who need skilled care but can safely remain at home.
Is home health the same as home care?
No. Home health provides skilled medical care prescribed by a physician, while home care focuses on non-medical assistance such as personal care, meal preparation, companionship, and help with daily activities.
Who qualifies for home health care?
Eligibility is determined by your physician and depends on your medical condition, need for skilled services, and whether home-based care is appropriate for your recovery or ongoing treatment.
Do I need a physician referral?
In most cases, yes. Home health services generally require a physician's order confirming that skilled care is medically necessary and outlining the services you need.
How quickly can services begin?
Start times vary depending on your physician's orders, insurance authorization, and clinical needs. In many situations, services can begin within a few days after the referral is received.
What services are included in home health care?
Services may include skilled nursing, physical therapy, occupational therapy, speech therapy, wound care, medication management, chronic disease monitoring, and patient or caregiver education.
Will a nurse visit my home?
Yes, if skilled nursing is part of your care plan. Your nurse will monitor your condition, provide treatments, manage medications, educate you and your caregiver, and communicate with your physician as needed.
Do you provide therapy services?
Yes. Depending on your physician's orders, physical, occupational, and speech therapists work with you to improve strength, mobility, communication, and independence during recovery.
How are caregivers involved?
Family members and caregivers are important partners in the recovery process. We provide education, practical guidance, and ongoing support so they feel confident assisting between visits.
Will I receive instructions for care at home?
Yes. Our team provides clear education on medications, exercises, wound care, safety precautions, and other recommendations to help support a safe recovery and reduce the risk of complications.
Is home health covered by Medicare?
Medicare generally covers eligible home health services when they are medically necessary and ordered by a physician. Coverage depends on meeting Medicare's eligibility requirements.
Does private insurance cover home health?
Many private insurance plans and Medicaid programs also cover home health services. Coverage varies by plan, benefits, and medical necessity, so our team can help verify your benefits.
